Northern Home Care’s goal is to provide appropriate medical and non-medical care and services that assure wellness, safety and comfort in the home, hospital, assisted living, rehabilitation, and nursing home settings. Our experienced and dedicated teams of healthcare professionals are available 24 hours a day and provide individualized support as needed on a hourly, daily, or weekly basis. It begins with an assessment. A professional nurse assesses each individual to determine the level of care required and evaluate any risk factors. A client may be assessed at no charge or obligation in his/her home or in a facility.
After the initial assessment a care plan and/or treatment plan is developed in conjunction with the physician, the patient, family, caregivers, case managers, and payer source based on the health needs. Patient care is monitored daily either through direct supervision, phone contact, or review of records and reports. Information is communicated to physicians, case managers, family members, and Northern’s staff to ensure the highest quality of care at the appropriate level for all of our patients and their families.
Northern Home Care provides a unique program for the residents of the Springs at FountainView. Our licensed aides are on-site, around the clock to provide assistance with all activities of daily living. The aides will provide services based on the residents individualized contracted level of care. This program enables Northern Home Care to provide a cost effective means to meet the varied needs of the residents of the Springs at FountainView.
